• Direct Responsible Charge (DRC) Form
  A completed Direct Responsible Charge (DRC) form must be submitted with all applications for Level III and Level IV Water Treatment & Wastewater Treatment examinations.
  
  
• Continuing Education Unit (CEU) Credit Form
  A completed Continuing Education Unit (CEU) Credit Form must be submitted by certified operators to have training time credited. This records their continuing education requirements to retain certification.
