• Wastewater Collection OIT, Levels I to IV
• Wastewater Treatment OIT, Levels I to IV
• Industrial Wastewater Treatment Levels I to IV

Small System Application for Certification Form

For operators applying to write any of the following exams:

• Small Water Systems
• Small Wastewater Systems – Lagoons
• Small Wastewater Systems – Mechanical
• Bulk Water Delivery

Direct Responsible Charge (DRC) Form

A completed Direct Responsible Charge (DRC) form must be submitted with all applications for Level III and Level IV examinations.

Continuing Education Unit (CEU) Credit Form

A completed Continuing Education Unit (CEU) Credit Form must be submitted by certified operators to have training time credited. This records their continuing education requirements to retain certification.
